SCHOLARSHIP CHECKLIST
Before sending in your application for scholarship, please check to be sure you have included all of the following:
TO QUALIFY:

___must be currently enrolled in an accredited graduate level program leading to a Master’s degree or Post Master’s certification as an Advanced Practice Nurse
OR
-
___have completed a Master’s level Advance Practice Nurse program or Post-Master’s certificate program as an Advance Practice Nurse and currently be enrolled in a PhD in Nursing or a DNP program.

___Program of study for the Master’s degree or the Post-Master’s Certificate must be located within the state of Missouri.

___Student must have completed at least 12 hours of credit toward Master's degree or Post-Master's Certificate.

___The program of study for the PhD in Nursing or DNP does not have to be located in the state of Missouri.

___ Student enrolled in a program of study for a PhD in Nursing or a DNP must be a resident of Missouri.

___why you feel you should be considered for this scholarship
