The purpose of this position is to serve as a core member of the MO HealthNet Health Home and Patient Centered Medical Home (PCMH) teams and to be intimately involved with all aspects of these programs. The RN will promote overall patient health by providing, delegating, evaluating and coordinating comprehensive professional nursing care.
